Sometimes it becomes necessary for a person to give a declaration that they are the same person as another name (i.e. in the event the person may have been know by or used different names)
This package includes two (2) different types of Name Affidavit forms that allow a person to declare under penalty of perjury or under oath that he/she is the same person as the other names listed in the affidavit. This form is similar to a Declaration under Oath. (1) One is an Affidavit under penalty of perjury that the Declarant (i.e. person signing the affidavit) is the same person as the other name listed in the affidavit. (2) The second form is a Name Affidavit under oath where the notary states that he/she gave the Declarant (the person making the declaration) an oath and that under oath, the Declarant stated that he/she is the same person as the other name listed on the affidavit. This form can be used in Alabama. This package includes (1) Instructions and Checklist; (2) Name Affidavit under penalty of perjury; (3) Name Affidavit under oath. |
